Instructions For Completing the MHSAA Broadcast Application:

The Broadcast Application Form is provided in three formats:
-- Microsoft Word
-- Rich Text Format (Which can be untilized by most types of popular word processing programs)
-- PDF

· You may open any form online. You can only save the Word and Rich Text Format files.

· Be sure to provide all information requested. It is especially important that radio stations list their call letters.

· Be sure you list the proper rights fee. Consult the MHSAA Broadcast Regulations for rates.

· If you have submitted paperwork to the MHSAA since August 1, 2005 to join the MHSAA Championship Radio Network, you are eligible for a partial or full fee waiver, and may check the appropriate box.

· If you are an MHSAA member school, and your school or school district owns and operates its own audio/video outlet, you may be eligible for a full fee waiver. Click here to obtain a form to submit to the MHSAA office for consideration. Public access and local origination channels and those outlets which are not operated by the school district with school district employees, and which do not carry school programming exclusively, may not be eligible for this waiver. This form must be submitted annually by the member school principal, and approval will not be granted to broadcast under this waiver until it is submitted to the MHSAA Office.

· To fill out the Broadcast Application, place the cursor in each box and type the requested information. You cannot use the TAB key to move from field to field.

· You must submit a separate application for each game you wish to cover in football if you're a radio outlet, and each game you wish to cover in all sports if you're a television outlet. You are not allowed to submit an application for Two Or More Games At One Site when covering football games or televising any game.

· When you are finished filling out the Word or Rich Text form, save it to your computer, launch your own e-mail program, attach the form to an e-mail and send it to the MHSAA Office at broadcast@mhsaa.com.

· PDF forms may be used only in those cases where an outlet cannot use the Word or Rich Text forms, and submitted by fax to 517.332.4071. PDF users should open and fill out the form on line, print a copy, and then submit by fax.

· A few reminders regarding financials:
-- If you are selling tapes from a game, prepayment is required before your application will be approved.
-- Stations which do not originate games after applying for them must contact the MHSAA office to avoid being charged for those games.
-- The MHSAA will invoice stations the week after all tournaments for that season are completed.
